I/we agree to pay the remaining balance of $________ on or before _________________________, ____. If the remaining balance is not paid by the due date, my/our reservation may be canceled and all money previously paid forfeited. 

All payments are due by the dates specified in this agreement. If payments are late, we reserve the right to rebook the time you have reserved and to retain any deposit received. 

I/we agree that if I/we cancel our reservation, the reservation deposit will be forfeited. If it is necessary to change the date and/or time of the wedding or event, and the new time is available at Foothills Chapel, the reservation deposit will apply toward the new time if notice is given six months prior to the change. If notice is given three to six months prior to the change, the reservation deposit will apply toward the new time only if the original time is rebooked. If notice of the change is given less than three months prior to the change, the original reservation deposit will be forfeited. If the reservation is canceled after full payment is made, and the reserved time is rebooked for another event, 50% of the amount received will be refunded. If another event is not booked for that time, the full amount will be forfeited.

Damage Deposit Agreement  

I/we agree to pay Foothills Chapel a $200 damage deposit on or before ____________    for use of the facility at the above date and time. The deposit will be refunded on or before ____________- if the following policies and procedures have been observed.

1.) Punctuality. There may be other events preceding and/or following yours. Please inform all clergy, musicians, florists, photographers, assistants, guests and the wedding party of the rehearsal and wedding times, and do not allow them to arrive prior to your scheduled time, Start your dean-up 15 minutes before the end of your reserved time so that the chapel and dressing rooms are available for the next party.

2.) No smoking inside the building. Smoking is permitted in the parking lot and by front door only. Place all matches and cigarette butts in the ashtrays, not on the grounds.

3.) No rice, confetti, seeds, streamers or "spaghetti" anywhere in the building or on the premises. Rice hurts birds; today's seeds are tomorrow's weeds, You may have your guests throw birdseed only if you are willing to pay us $50 to hire someone to clean them up. Bubbles are recommended instead!

4.) Decorations. Do not move our flowers, candles, organ, piano, etc,: let us do it! Use plastic pew clips only for pew decorations. If your flower girl will be throwing fresh petals, please use a runner to cover the carpet. Otherwise, use silk flowers or feathers. Do not rearrange, add to or take flowers from our silk flower arrangements, or alter pew bows, candelabras, and other decorations; only our florist is authorized to alter them. We will remove our decorations it you plan to use your own flower arrangements or pew bows. Candles must conform to all Golden Fire Department regulations.

5. Food and Drink. No food or drink anywhere in the building except in the reception hall and kitchen. This includes all beverages in the chapel lobby and dressing rooms.

6.) Babies/Small Children. We do not permit any food or drink in the chapel under any circumstances. Please do not bring food or drink into the chapel, even to feed babies. If babies or small children must be fed or changed, this must be done in the small side chapel for health reasons and to protect our carpets and pew cushions. Change baby's diapers on the pews in the small side chapel or in the rest rooms.

I/we have read and agree to abide by the policies and procedures of Foothills Chapel. Foothills Chapel reserves the right to cancel and terminate any event at any time if the use of the facility and/or the conduct of guests or outside service personnel violates these policies and procedures. In the event of a cancellation under such circumstances, all or part of the damage deposit will be forfeited.
